Job summary

Essentia Health's South University Clinic seeks a Home Health Nurse to assess, plan, implement, evaluate and coordinate home care for patients under nursing standards and clinic policies. You will provide direct patient care for medical, diagnostic and emergency conditions and work in day rotation with on-call weekends.

Minimum 1 year of clinical nursing experience is required. Current nursing licensure in the state of employment and BCLS certification (or eligibility) are required.

Qualifications

• Minimum of 1 year clinical nursing experience.
• Current nursing licensure in the state of employment; multi-state licensure may be required.
• BCLS certification or ability to obtain within 1 month of hire.

Responsibilities

Job Description:

Responsible to assess, plan, implement, evaluate, supervise and coordinate the home care of patients in accordance with professional standards of nursing and policies and procedures of Essentia Health. Provides direct patient care to patients requiring care for medical, diagnostic and emergency conditions.

Work Experience:
East Market: Minimum of 1 year clinical nursing experience.

Education as need for state licensure.

• Detroit Lakes and Graceville: Must meet the driving requirements and criteria acceptable to Essentia Health’s insurer
