Job summary

Transition Home Healthcare in Houston, TX is seeking a dedicated Registered Nurse (RN) for Home Health to join our growing team. This role covers North, Northwest, Southwest, and Inner Loop areas with PRN and Full Time shifts available.

You will coordinate with physicians, therapists, and caregivers to plan, implement, and monitor patient-centered home health care, educate families, and maintain accurate clinical notes. Competitive pay, benefits, and mileage reimbursement are provided.

Qualifications

• Valid RN license in Texas or compact license.
• Home health experience of at least 1 year.
• Knowledge of home health regulations and compliance.
• Experience with HCHB EMR preferred.
• Excellent communication and people skills.
• Ability to work independently and as part of a team.

Responsibilities

• Oversee nursing care team.
• Collaborate with physicians and therapists for cohesive patient care.
• Plan, organize and direct home care services.
• Assess patient care needs and develop care plans.
• Educate patients and families on managing health conditions and medications.
• Prepare timely, complete clinical notes.

Responsibilities

• Oversee nursing care team.
• Communicate and collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive and comprehensive patient care.
• Plan, organize, and direct home care services.
• Conduct assessments of patient care needs and develop care plans.
• Educate patients and their families on managing their health conditions, medication regimens, and any necessary lifestyle changes.
• Prepare timely, complete, and accurate clinical notes.

Qualifications

• Valid RN license in the State of Texas or compact license.
• Home health experience (at least 1 year).
• Knowledge of home health regulations and general healthcare compliance.Experience with Home Care Home Base (HCHB) EMR (preferred).
