Transition Home Healthcare in Houston, TX is seeking an LVN for home health visits. PRN and full-time shifts available, covering Southwest and Inner Loop areas.

You will develop individualized care plans, perform nursing visits, and document clinical notes, while educating patients and families and ensuring compliance with agency policies. Qualifikationen

• Valid and current LVN license in Texas or compact state.
• 1+ year of home health experience preferred.
• Knowledge of home health regulations and compliance.

Responsibilities

• Collaborate with the home healthcare team to develop and implement individualized care plans.
• Complete home care nursing visits.
• Prepare timely, complete, and accurate clinical notes.
• Communicate with healthcare providers and other care team members.
• Educate patients and their families about health conditions, treatment plans, and self-care techniques.
• Adhere to all agency policies, procedures, and regulatory requirements related to home health care.

Qualifications

• Valid and current LVN license in the state of Texas or compact state.
• Minimum of one (1) year of experience in a home health setting
• Knowledge of home health regulations and general healthcare compliance.
• Experience with HCHB EMR. (preferred but not required)
• Excellent communication and people skills to effectively interact with patients, families, and healthcare team members.
• Compassionate and empathetic demeanor with a commitment to providing patient-centered care.
