Job summary

Transition Home Healthcare, an elite home health agency serving Houston, TX and surrounding areas since 2010, is growing and seeking a dedicated Registered Nurse (RN) - Home Health.

As an RN on our team, you will oversee nursing care, coordinate with physicians and therapists, plan and direct home care services, and educate patients and families to manage health conditions. PRN shifts available.

Qualifications

• Valid RN license in Texas or compact license.
• Home health experience of at least 1 year.
• Knowledge of home health regulations and general healthcare compliance.
• Experience with HCHB EMR (preferred).
• Excellent communication and people skills are essential.
• Ability to work independently and as part of a team.

Responsibilities

• Oversee nursing care team.
• Communicate and collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive and comprehensive patient care.
• Plan, organize, and direct home care services.
• Conduct assessments of patient care needs and develop care plans.
• Educate patients and their families on managing health conditions, medication regimens, and any lifestyle changes.
• Prepare timely, complete, and accurate clinical notes.

Shifts Available: PRN

Service Area for Coverage: North, Northwest, Southwest, Inner Loop

Responsibilities

• Oversee nursing care team.
• Communicate and collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive and comprehensive patient care.
• Plan, organize, and direct home care services.
• Conduct assessments of patient care needs and develop care plans.
• Educate patients and their families on managing their health conditions, medication regimens, and any necessary lifestyle changes.
• Prepare timely, complete, and accurate clinical notes.
