Job summary

Emplify Health, a partnership of Bellin Health and Gundersen Health System, is seeking a Registered Nurse for our Home Dialysis team. You will provide specialized care to hospitalized patients with kidney disease and perform home visits, coordinating dialysis across peritoneal and hemodialysis modalities.

Requirements include an Associate Degree in Nursing and WI RN licensure at hire, with MN/IA licensure possible within 3 months. On-call duties include every 3rd weekend and holiday.

Qualifications

• Must hold RN licensure in Wisconsin upon hire.
• RN licensure in Minnesota and Iowa within 3 months of hire.
• Basic Life Support for Healthcare Providers.

Responsibilities

• Provide hospital and home visits to patients with kidney disease.
• Assist with acute dialysis as needed and coordinate with peritoneal/hemodialysis care.
• Travel to patient homes is required.
• 36-hour work week; 0.9 FTE (72 hours every two weeks); 7:00am–4:30pm, four days per week; on-call every 3rd weekend/holiday.

Join our Home Dialysis team as a Registered Nurse and provide specialized care to hospitalized patients with kidney disease. In this role, you’ll complete hospital and home visits, assist with acute dialysis as needed, and work closely with patients receiving peritoneal dialysis and hemodialysis. Travel to patient homes is required.

What you will do:

• 0.9 FTE = 72 hours/every two weeks
• 7:00am-4:30pm, four days per week
• On-call every 3rd weekened and holiday

What you need:

• Minimum of Associate Degree in Nursing
• RN licensure to practice in the state of Wisconsin upon hire
