Job summary

BrightStar Care of Concord, Lexington & Woburn is seeking an experienced Registered Nurse (RN) to lead clinical care in home health settings. You will develop and implement individualized patient care plans and supervise care-staff to deliver high-quality services.

The role requires an active RN license, CPR certification, 3–5 years of clinical experience with at least 1 year in home health or similar settings, and strong IV therapy, wound care, and G-tube skills.

Qualifications

• Active RN license is required.
• CPR certification is required.
• Three to five years of clinical experience with one year in a home health agency or similar setting preferred.
• Strong IV therapy, wound care, and G-tube care skills.

Responsibilities

• Lead and manage clinical care-staff for assigned business
• Develop and implement individualized patient care plans
• Perform in-home assessments and ensure quality care delivery
• Administer medications, IV infusions, and treatments as prescribed
• Provide advanced wound care, including Wound VAC (NPWT) management
• Maintain accurate documentation and ensure regulatory compliance
• Supervise and support nurses, aides, and caregivers
• Coordinate staffing and ensure proper coverage for patient cases
• Collaborate with physicians and healthcare professionals
