Job summary

BrightStar Care of Concord, Lexington & Woburn is seeking passionate Registered Nurses (RNs) to deliver exceptional home health care and to grow in a supportive, high-quality environment.

The role offers base salary with annual bonus, plus a $2,000 signing bonus and performance-based incentives. You will lead clinical care, develop care plans, and collaborate with a multidisciplinary team to ensure high-quality patient outcomes.

Qualifications

• Active RN license required.
• CPR certification required.
• 3–5 years of clinical experience; 1 year in home health, primary care clinic, or health facility preferred.
• Strong IV therapy, wound care, and G-tube care skills.
• Reliable, detail-oriented, and passionate about patient care.
• Reliable car and auto insurance.

Responsibilities

• Lead and manage clinical care and care-staff for assigned cases.
• Develop and implement individualized patient care plans.
• Perform in-home assessments and ensure quality care delivery.
• Administer medications, IV infusions, and treatments as prescribed.
• Provide advanced wound care, including NPWT as applicable.
• Maintain accurate documentation and ensure regulatory compliance.
• Supervise and support nurses, aides, and caregivers.
• Coordinate staffing and ensure proper coverage for patient cases.
• Collaborate with physicians and healthcare professionals.
