Job summary

Shreveport Rehabilitation Hospital is seeking a registered nurse to deliver direct and indirect patient care in an inpatient rehabilitation setting. You will use the nursing process for assessment, planning, intervention, and evaluation while coordinating with patients, families, and an interdisciplinary team.

The role requires current RN licensure and BLS certification. You will contribute to discharge planning and support safe transitions of care, with focus on communication and timely care

Qualifications

• Current state licensure as a Registered Nurse.
• BLS certification from AHA and/or Red Cross (online certification not acceptable).
• Experience in communicating clinical changes to physicians and team.
• Ability to support discharge planning and transitions of care.

Responsibilities

• Provide direct and indirect patient care using the nursing process (Assessment, Planning, Intervention, Evaluation).
• Develop individualized care plans with patients, families, and interdisciplinary staff.
• Communicate clinically significant changes to physicians, nursing supervisor/manager, and team.
• Engage in discharge planning to ensure safe, effective transitions of care.

Job description

Provides planning and delivery of both direct and indirect patient care through the comprehensive nursing process of Assessment, Planning, Intervention, and Evaluation. Develops individualized nursing care plans in coordination with patients, families, and interdisciplinary staff as appropriate. Communicates clinically significant changes in patient condition to Physicians, Nursing Supervisor/Manager, and team members as needed. Engages in and supports the discharge planning process to help ensure safe and effective transitions of care.

Pay:

Rate of pay is based on years of experience and qualifications.

Minimum Qualifications:

• Current state licensure as aRegistered Nurse
• Basic Life Support (BLS)certification from the American Heart Associationand/or American Red Cross (online certification is not acceptable)

Desired Qualifications:

• Experience providing care in an inpatient rehabilitation setting
