Job summary

Interim HealthCare is seeking a dynamic Home Health Registered Nurse (RN) to join our team serving Dorchester, Charleston, and Berkeley Counties. We offer strong support, work/life balance, and competitive pay with 1:1 patient ratios.

As a Home Health RN, you will perform comprehensive patient assessments, develop and evaluate plans of care, educate patients and families, and coordinate with physicians and staff to improve patient outcomes.

Qualifications

• Graduate of an accredited school of nursing and an active SC RN license.
• 2 years' nursing experience with 1 year in home health preferred.
• New graduates are welcome.

Responsibilities

• Participates in ongoing interdisciplinary assessment of the patient.
• Completes initial and ongoing comprehensive assessments of the patient’s needs.
• Participates in the development and evaluation of the plan of care in partnership with the patient, representative, and caregiver(s).
• Participates with the patient in clarifying mutually agreed upon goals and incorporates these goals into the plan of care/service plan.
• Provides services that are ordered by the physician as indicated in the plan of care.
• Counsels and educates the patient and family in meeting healthcare-related needs.
• Initiates appropriate preventative and rehabilitative nursing procedures.
• Monitors and evaluates outcomes of patient care.
• Communicates with appropriate staff regarding changes in the patient’s needs.
• Supervises paraprofessional staff.
• Effectively communicates with other members of the interdisciplinary healthcare team providing care to promote coordination of patient care.

• Participates in in-services or training, as requested.
• Accesses personal health information (“PHI”) necessary to fulfill the above duties and responsibilities.

• Graduate of an accredited school of nursing andcurrent RN License in SC or compact license.
