Job summary

Interim HealthCare seeks a committed Home Health RN to deliver skilled nursing care in patients' homes within Anderson County and surrounding areas. You will collaborate with physicians and the interdisciplinary team to support safe recovery and long-term health, performing comprehensive assessments and managing plans of care.

Requirements include an accredited RN program graduate and SC RN licensure (or compact).

Qualifications

• Graduate of an accredited RN program.
• Active South Carolina RN license or Compact License.
• Minimum one year of nursing experience.
• Home Health, Med-Surg, Critical Care, or Case Management experience preferred.
• Familiarity with OASIS documentation preferred.

Responsibilities

• Provide skilled, patient-centered home care and coordinate with physicians and the team.
• Perform comprehensive patient assessments and document accurately.
• Develop and adjust individualized plans of care.
• Monitor patient status and communicate changes promptly.
• Educate patients and families on medications, treatments, and home safety.
• Coordinate care with therapists and aides to ensure seamless service.
• Prepare patients for discharge and document care in the EMR.

Shift: Full-time, Friday 4:30pm — Monday 8:30am
Location: Piedmont, Pelzer, Williamson. Anderson County up to West Greenville.

• Perform comprehensive patient assessments, includingaccurateOASIS documentation
• Develop, implement, and adjust individualized plans of care
• Monitor patient condition and communicate changes promptly to physicians to prevent complications
• Educate patients and caregivers on medications, treatments, disease management, and home safety
• Coordinate care with therapists, aides, and other providers to ensure seamless, compliant care
• Prepare patients and families for safe, successful discharge and document care accurately in the EMR

WhatYou’llNeed

• Graduate of an accredited Registered Nurse (RN) program
• Active South Carolina RN license or Compact License
• Minimum one year of nursing experience
• Experience in Home Health, Medical Surgical, Critical Care, or Case Management preferred
