Job summary

Interim HealthCare of The Upstate is seeking a compassionate Registered Nurse (RN) to deliver skilled home health care across South Carolina. You will collaborate with physicians and a multidisciplinary team to support safe recovery and long-term health in patients’ homes.

You will conduct comprehensive assessments, document OASIS data, and tailor plans of care to individual needs. We offer flexible scheduling and a supportive environment focused on quality outcomes.

Qualifications

• Graduate of an accredited Registered Nurse (RN) program.
• Active South Carolina RN license or Compact license.
• Minimum one year of nursing experience.
• Experience in Home Health, Medical Surgical, Critical Care, or Case Management preferred.
• Familiarity with OASIS documentation preferred.

Responsibilities

• Provide skilled, patient-centered care in the home with physicians and the interdisciplinary team.
• Perform comprehensive patient assessments and accurate OASIS documentation.
• Develop, implement, and adjust individualized plans of care.
• Monitor patient condition and promptly communicate changes to physicians.

As a Registered Nurse (RN), you will:

• Provide skilled, patient centered care in the home while partnering with physicians and the interdisciplinary team to support safe recovery and long-term health


• Perform comprehensive patient assessments, includingaccurateOASIS documentation


• Develop, implement, and adjust individualized plans of care


• Monitor patient condition and communicate changes promptly to physicians to prevent complications


• Educate patients and caregivers on medications, treatments, disease management, and home safety


• Coordinate care with therapists, aides, and other providers to ensure seamless, compliant care


• Prepare patients and families for safe, successful discharge and document care accurately in the EMR
