Job Summary

Provides direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization.

Responsibilities
  • Implements and monitors patient care plans; monitors, records and communicates patient condition as appropriate.
  • Serves as a primary coordinator of all disciplines for well‑coordinated patient care.
  • Notes and carries out physician and nursing orders.
  • Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
  • Assessment – An RN uses a systematic, dynamic way to collect and analyze data about a patient, including physiological, psychological, sociocultural, spiritual, economic, and lifestyle factors.
    • Initial assessment
    • Re‑assessment
    • Focused assessment based on patient condition, setting and population
  • Problem Identification – Clinical judgment about the patient’s response to actual or potential health conditions or needs.
  • Planning and Outcomes – Sets measurable and achievable short‑ and long‑range goals in collaboration with the patient and family. The data, problems, and goals are documented in the patient’s care plan for all health professionals involved.
    • Establishing daily goals with the patient and family
    • Formulating a plan of care
    • Transition or discharge planning
  • Implementation – Nursing care is carried out according to the care plan, ensuring continuity of care for the patient during hospitalization and in preparation for discharge. Care is documented in the patient’s record.
    • Ensures patient safety
    • Medication administration
    • Provision of ordered treatments
    • Patient/Family education
    • Provision of nursing care – Documentation in healthcare record
  • Evaluation – Continuously evaluates the patient’s status and the effectiveness of nursing care, modifying the care plan as needed.
  • Care Coordination – Initiates actions with patients, families, and team members to manage and correct the sequence, timing, and effectiveness of patient care from admission to discharge.

Qualifications
  • Minimum Requirements:
    • Education: Graduation from an accredited School of Nursing. Continuing education requirements as defined within the CARE program.
    • Experience: One (1) year as an RN.
    • Licensure/Certifications:
      • Valid current Arizona Nursing License or compact state license and ability to become Arizona licensed within 30 days.
      • Basic Life Support (BLS)
      • American Heart Association and other certifications as required by individual department (Attachment A).
  • Preferred Requirements:
    • Education: Bachelor of Science in Nursing.
    • Licensure/Certifications:
      • Advanced Cardiac Life Support (ACLS)
      • American Heart Association
      • Pediatric Advanced Life Support (PALS)
      • Neonatal Resuscitation Program (NRP) or other certifications as required by individual department.
