Job summary

Crossroads Hospice & Palliative Care is seeking a Registered Nurse Case Manager to provide direct, compassionate care to patients in their homes or care facilities. You will listen to patients and families, coordinate with the interdisciplinary team, and advocate for patient wishes with a holistic approach.

The role emphasizes dignity, respect, and meaningful end-of-life care, with opportunities to grow within the organization and support patients and their families beyond medical treatment.

Qualifications

• Six months minimum skilled nursing experience.
• Active, unencumbered state RN license.
• Current driver's license and auto liability coverage.
• Interest and ability in providing patient care in homes and facilities.

Responsibilities

• Provide direct care to patients in their homes or care facilities.
• Build trusting relationships with patients and their families.
• Serve as primary communication link within the interdisciplinary team of holistic care providers.

Registered Nurse Case ManagerQualifications:

• Six (6) months minimum prior skilled nursing experience.

Registered Nurse Case Manager Schedule & Benefits:

• Monday to Friday, 8:30 AM to 5:00 PM
• No Mandatory on-call required
