Crossroads Hospice seeks a Registered Nurse Case Manager to deliver compassionate in-home and facility-based care. You will listen to patient needs, respect their wishes, and act as the primary liaison within our holistic care team while building trusting relationships with patients and families.

Qualifications

• Six (6) months minimum prior skilled nursing experience.
• Active, unencumbered state Registered Nurse license.
• Current driver's license with auto liability coverage.

• Provide direct patient care in patients' homes or care facilities.
• Collaborate with the interdisciplinary team to coordinate holistic care and communicate patient needs.
• Document patient assessments and care plans accurately to support continuity of care.

Registered Nurse Case Manager Qualifications

• Six (6) months minimum prior skilled nursing experience

Registered Nurse Case Manager Schedule & Benefits

• Monday to Friday, 8:30 AM to 5:00 PM
