Job summary

Crossroads Hospice & Palliative Care is seeking a Registered Nurse Case Manager to provide compassionate, in-home and facility-based care. You will assess patients, coordinate plan of care with an interdisciplinary team, and serve as the primary communication link between families and our care team.

We are dedicated to honoring patient wishes with dignity and respect, supporting families, and delivering personal end-of-life care.

Qualifications

• Six (6) months of skilled nursing experience.
• Active, unencumbered state RN license.
• Current driver’s license and auto liability coverage.
• Interest in providing patient care in homes and facilities.

Responsibilities

• Provide direct patient care in patients' homes or care facilities.
• Coordinate and implement individualized care plans with the interdisciplinary team.
• Act as primary liaison between patients, families, and the care team.
• Uphold dignity, compassion, and respect for patients at end of life.

As a Registered Nurse Case Manager, you will provide direct care to patients in their homes or care facilities. Registered Nurse Case ManagerQualifications:

• Six (6) months minimum prior skilled nursing experience.
• An active, unencumbered state Registered Nurse license.
• Current driver’s license and auto liability coverage.
• Interest and Ability in providing patient care in homes and facilities.

Registered Nurse Case Manager Schedule & Benefits:

• Monday to Friday, 8:30 AM to 5:00 PM
• No Mandatory on-call required
