Job summary

Crossroads Hospice & Palliative Care provides Registered Nurse Case Manager services delivering compassionate care in patients' homes and care facilities. You will be the trusted liaison among patients, families, and the interdisciplinary team, ensuring care respects patient wishes and dignified end-of-life support.

Qualifications

• Six (6) months minimum prior skilled nursing experience.
• An active, unencumbered state Registered Nurse license.
• Current driver's license and auto liability coverage.
• Interest and Ability in providing patient care in homes and facilities.

As a Registered Nurse Case Manager, you will provide direct care to patients in their homes or care facilities. Building trusting relationships with patients and their families is a cornerstone of this position, allowing you to serve as the primary communication link within our interdisciplinary team of holistic care providers.

Registered Nurse Case ManagerQualifications:

Registered Nurse Case Manager Schedule & Benefits:
