Job summary

Jefferson Regional Specialty Hospital, part of Lifepoint Health, is seeking a Charge Nurse for our Rehabilitation Services Unit. You will lead an RN team in delivering safe, compliant care to patients recovering from stroke, brain injury, and orthopedic or spinal conditions, with a focus on improving independence.

You will mentor staff, oversee documentation, and coordinate assignments to meet patient needs and acuity while supporting continuous quality improvement efforts across the unit.

Qualifications

  • Current state RN license (or compact state) is required.
  • Associate's degree in nursing is required; BSN preferred for advancement.
  • AHA Basic Life Support (BLS) certification is required.
  • Minimum of two years in a rehabilitative or acute care setting.

Responsibilities

  • Provide clinical leadership and mentorship to nursing staff.
  • Oversee documentation accuracy and compliance; coordinate staff assignments by patient needs and acuity.
  • Foster open communication and professional growth within the team; ensuring patient safety and care standards.
  • Participate in CQPI and ongoing quality improvement initiatives.
  • Assist with assignments based on patient acuity and staff competency; support discharge planning as needed.

Schedule: 3x12s Days (7p-7a)

Jefferson Regional Rehabilitation Services Unit is a 40-bed acute inpatient rehabilitation facility where compassionate care, teamwork, and patient-centered recovery are at the heart of everything we do. As a Charge Nurse, you will lead and support a dedicated team of Registered Nurses (RNs) in delivering high-quality, safe, and compliant care to patients working toward regaining strength, independence, and quality of life.


In this role, you will provide clinical leadership and mentorship to nursing staff, oversee documentation accuracy and compliance, and coordinate staff assignments based on patient needs and acuity. Working alongside a collaborative and supportive team, you will help foster an environment of open communication, professional growth, and excellence in patient care. Together, we are committed to helping patients achieve their highest level of independence while supporting one another in delivering exceptional care every day.


How You'll Contribute

A Rehab RN who excels in this role:



  • Complete routine audits to ensure accurate and timely documentation in Cerner and UDS for compliance.

  • Identify and address educational needs among nursing staff based on clinical practice and feedback.

  • Educate and monitor staff on proper QI documentation to reflect accurate burden of care.

  • Serve as a clinical mentor and resource for nursing staff caring for medically complex patients.

  • Support patient and family education and coordinate resources to reduce hospital readmissions.

  • Provide UDS documentation/orientation training for newly hired licensed nurses.

  • Conduct remediation sessions for nurses with documentation deficiencies.

  • Participate actively in Continuous Quality and Performance Improvement (CQPI) efforts.

  • Ensure the nursing team delivers care that maximizes patient safety and meets all care standards.

  • Assist with nursing staff assignments based on patient acuity and staff competency.

  • Perform other duties as assigned.

What We're Looking For

Applicants should have a current state or compact state RN license and possess an associate's degree from an accredited nursing school. Additional requirements include:



  • Certifications: AHA Basic Life Support (BLS) Requried.

  • Experience: Minimum of two years in a rehabilitative or acute care setting.

  • Licenses: Must have a current state or compact state RN license.

  • Education: Associate's degree required. BSN preferred.

  • Previous Leadership Experience Preferred.
