Job summary

Phoenix Home Care & Hospice in Joplin, MO is seeking an experienced RN Case Manager to provide compassionate nursing care to clients with a terminal diagnosis. You will collaborate with the interdisciplinary team to develop and implement a patient/family centered plan of care and guide goals of care discussions.

The ideal candidate demonstrates energy, motivation, strong organizational and interpersonal skills, and the ability to offer clear guidance in demanding situations, supporting learning

Qualifications

• Missouri or Compact RN license required.
• Valid driver’s license and auto liability insurance.

Responsibilities

• Provide education to patients/family regarding plan of care and healthcare decisions.
• Consult with attending physician and Hospice Medical Director on changes to condition or plan.
• Coordinate hospice care with the team and patients/family to meet needs.
• Conduct initial and ongoing comprehensive assessments.
• Perform hospice care evaluations and informational visits.

Phoenix Hospice RN Case Managers provide compassionate nursing care to clients and their families dealing with a terminal diagnosis. Will work with the interdisciplinary team to develop and implement a plan of care that is patient/family centered, with goals of care discussions and guidance with healthcare decisions to give quality of life.

Responsibilities

• Provide education to patients/family members regarding the process, plan of care, and offers guidance with healthcare decisions.
• Consult with the attending physician and Hospice Medical Director concerning changes in patients condition requiring order changes and/or changes to plan of care.
• Coordinate hospice care by working with other team members as well as with the patients/family members to ensure needs are met.
• Conducts initial as well as on-going comprehensive assessments.
• Perform evaluations for hospice care along with informational visits.
