Job summary

Adena Hospice is seeking a Registered Nurse Case Manager to assume leadership for patient assessment and care planning. The role includes case management, direct clinical care, and coordination across disciplines under physician direction.

The RN must have an active license in the state of employment and at least one year of RN experience in acute or home care settings. Ability to remain calm, compassionate, and patient-focused is essential.
