Job summary

Adena Hospice is seeking a Registered Nurse Case Manager (Hospice) to lead patient assessments, plan of care, and coordination across disciplines under physician direction. The RNCM ensures quality, cost-effective, and billable care with direct patient care and supervision.

Qualified candidates will have at least 1 year of RN experience in acute or home care, active RN license in the job state, and a Hospice-friendly approach; on-call rotations and schedule support are included.

SCHEDULE: Monday - Friday, 8:00am - 4:30pm; On-Call Rotation every 20th weekend; 1 holiday every 3 years

The Registered Nurse Case Manager (RNCM) - Hospice assumes ultimate accountability and leadership for the assessment of the patient condition and plan of care. The RNCM provides case management, clinical care and is responsible for coordination all disciplines involved in providing quality, cost-effective and billable care through direct care and supervision of care under the direction of the physician. ASHN adopts the RN scope of practice as outlined by the state for its licensed RN personnel.

QUALIFICATIONS

• One year of experience as a RN in an acute care or home care setting is required.
• Hospice experience is preferred.
• Maintains active RN license in state of employment
• Undertakes and practices agency policies and procedures
• Maintains a professional appearance as a representative of the company
