Job summary

Adena Hospice in Chillicothe, OH is seeking a Registered Nurse Case Manager to lead patient assessments and develop plans of care for hospice patients. You will coordinate care across disciplines and supervise direct care under the physician’s direction.

The RNCM provides case management, clinical care and is responsible for coordination all disciplines involved in providing quality, cost-effective and billable care through direct care and supervision of care under the direction of the physician.
