Job summary

Affinity Hospice is seeking a Registered Nurse/Case Manager to lead clinical care for patients and families in home-based hospice settings across Georgia. You will coordinate an interdisciplinary team, develop care plans, and ensure documentation and infection control standards are met.

Qualifications

• Graduate of an accredited nursing program.
• Current RN license in state of Georgia.
• Current CPR certification.
• Minimum of two years RN experience in clinical care, home health or hospice preferred.
• Must have a valid driver's license with insured vehicle.
• Excellent communication, problem solving and math skills.

Responsibilities

• Case manages and provides clinical direction to the interdisciplinary team, physician, and family.
• Develop and document the patient plan of care.
• Provide care utilizing infection control measures.
• Ensure continuity of quality patient care with proper documentation.
• Monitor cases to meet third-party payer requirements.
• Assume on-call responsibilities as assigned.
• Supervise hospice aides and licensed practical nurses.
• Perform other nursing duties as assigned by management.
