Job summary

Adena Hospice is seeking a Registered Nurse Case Manager to lead the assessment and plan of care for hospice patients. You will coordinate clinical care, supervise disciplines, and ensure cost-effective, billable services under physician direction.

Requirements include at least one year of RN experience in acute or home care, an active RN license in the state, and a compassionate, patient-centered approach. Hospice experience is preferred; valid driver’s license and auto insurance are required.
