Job summary

Affinity Hospice is seeking an experienced RN Case Manager to plan, organize, and direct hospice care for patients in their homes and communities. You will lead the interdisciplinary team to implement care plans and ensure high-quality services while maintaining thorough documentation.

The ideal candidate has an RN license, CPR certification, and at least two years of hospice or home-health nursing experience.

Qualifications

• Graduate of an accredited nursing program.
• Current State license as a Registered Nurse (RN).
• Current CPR certification.
• Minimum of two years of RN experience in clinical care, home health, or hospice.
• Must have a valid driver's license with an insured vehicle.
• Excellent observation, verbal and written communication, problem-solving and math skills.
• Able to lift and transfer patients and equipment.

Responsibilities

• Case manages and provides clinical direction to the interdisciplinary team, physician, and family.
• Implement and document patient plan of care.
• Provide care with infection control measures to protect staff and patient.
• Ensure continuity of quality patient care with proper documentation.
• Monitor cases for compliance with third-party payer requirements.
• Lead supervision of hospice aides and licensed practical nurses.
• Perform on-call duties as assigned by management.
