Job summary

Beaumont Hospice is seeking a Hospice Registered Nurse Case Manager (RNCM) to coordinate care for patients at the end of life. You will work within an interdisciplinary team to ensure compassionate, patient- and family-centered care, guiding families through difficult decisions with empathy and expertise.

Qualifications

• RN license required.
• Experience in hospice or palliative care preferred.
• Ability to work in an interdisciplinary team.

Using hospice best-practice care delivery approaches, you will have all the tools and resources needed to ensure an excellent end-of-life experience for your patients.
