Job summary

St. Croix Hospice in Bemidji, MN is seeking a compassionate Registered Nurse Case Manager to guide patients and families through end-of-life care.

You will perform initial and ongoing assessments, develop individualized care plans, and lead symptom management with empathy and clinical excellence. Educate patients and caregivers, administer medications as prescribed, rotate on-call duties, and coordinate seamless transitions with the care team.

Qualifications

• RN license required.
• 1+ years of acute care experience; hospice/home health experience preferred.
• Active driving license and reliable transportation required.

Responsibilities

• Perform initial and ongoing comprehensive assessments to determine hospice needs.
• Develop individualized care plans with patients and families; update as needs evolve.
