Job summary

St. Croix Hospice is seeking a compassionate Registered Nurse Case Manager to provide 1:1, whole-person care in patients' homes.

You will partner with an integrated care team to coordinate individualized care plans and support families through difficult decisions with empathy and professionalism. The role requires an RN with an accredited degree and 1+ year of acute care experience; hospice/home health experience preferred.

Qualifications

• RN license required; accredited nursing program completed (Associates/Bachelors/Masters).
• 1+ years acute care experience; hospice/home health experience preferred.

Responsibilities

• Perform initial and ongoing comprehensive assessments to determine hospice needs.
• Develop individualized care plans with patient and family, updating as needs evolve.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques, medications, diet, and symptom management.
• Rotate on-call duties and ensure seamless care transitions.
