Professional Case Management is seeking a Registered Nurse - RN Case Manager to join our Tennessee team. In-home visits for former Nuclear Weapons Workers, conducted weekly and lasting 1–2 hours, with you choosing your schedule.

You will perform vitals, home safety checks, medication planning, and oversee caregivers to meet client needs. Competitive pay, benefits, and scheduling flexibility are offered.

Qualifications

• Graduate of a state approved school of professional registered nursing.
• Current, unrestricted RN license in the state of practice.
• Minimum of two years nursing experience including one year in home care or related field.
• One year of supervisory or case management experience preferred.
• Current CPR certification.
• BSN preferred for this role.

Responsibilities

• Provide direct care and case management of assigned clients.
• Plan, implement, and evaluate care provided to clients.
• Coordinate and participate in client care conferences as needed.
• Serve as on-site clinical resource and support to home care staff.
• Assist staffing coordinators with scheduling and scope of practice decisions.
• Ensure timely and complete clinical documentation and reporting.

The nursing visits are conducted weekly and typically last 1-2 hours per visit.
