Professional Case Management is seeking a Registered Nurse Case Manager to provide in-home nursing visits in Texas. This role focuses on direct patient care, care coordination, and family support for former nuclear workers with chronic illness.

You will conduct weekly visits, assess vitals, ensure home safety, and supervise home care staff. The position offers a flexible schedule and meaningful patient relationships, with weekly pay and competitive hourly wages.

Qualifications

• Graduate of a state approved school of professional registered nursing.
• Current, unrestricted RN license in the state of practice.
• Minimum of two years nursing experience including one year in home care.
• Current CPR certification.

Responsibilities

• Provide direct care and case management of assigned clients.
• Plan, implement, and evaluate care provided; coordinate client care conferences.
• Supervise and collaborate with caregivers to meet home care needs.

All our clients are former Nuclear Weapon's Workers with chronic/terminal illness due to exposure to toxic substances while working for the Government. The nursing visits are conducted weekly and typically last 1-2 hours per visit.
