Job summary

Professional Case Management (PCM) is seeking Registered Nurse Case Managers in Ohio to provide in-home nursing visits for former Nuclear Weapons Workers with chronic and terminal illnesses. Visits are weekly, typically 1–2 hours, including vitals, home safety checks, and medication planning, with supervision of caregivers as needed.

You will support care planning, collaborate with the regional director, and help ensure high-quality home care services across the client population.

Qualifications

• Graduate of a state approved school of professional registered nursing.
• Current, unrestricted RN license in the state(s) of practice.
• Minimum of two (2) years nursing experience including one (1) year in home care or closely related field.
• One (1) year of supervisory and/or case management experience preferred.
• Current CPR certification.

Responsibilities

• Provide direct care and case management of assigned clients.
• Plan, implement, and evaluate care provided.
• Coordinate and lead client care conferences as needed.
• Assist staffing coordinators regarding staffing and scheduling within scope of practice.
• Participate in performance improvement initiatives and clinical documentation reviews.

All our clients are former Nuclear Weapon's Workers with chronic/terminal illness due to exposure to toxic substances while working for the Government. The nursing visits are conducted weekly and typically last 1-2 hours per visit.
