Job summary

Professional Case Management is seeking a Senior RN-Case Manager in Kentucky to lead a team of RN Case Managers and deliver home-based care under state regulations. This role requires a current RN license, BSN preferred, and at least two years of nursing experience including one in home care, with supervisory experience preferred.

The position offers a strong work/life balance, benefits, and an hourly rate of $44 with possible shift differentials.

Qualifications

• Graduate of a state approved school of professional registered nursing.
• BSN preferred.
• Current, unrestricted RN license in the state(s) of practice.
• Minimum of two years nursing experience including one year in home care or closely related field.
• One year of supervisory and/or case management experience preferred.
• Current CPR certification.

Responsibilities

• Provide direct care and case management of assigned clients.
• Assist and collaborate with the regional director and other personnel to identify and correct issues and/or improve services.
• Plan, implement, and evaluate care provided.
• Coordinate and manage client care conferences as needed.
• Serve as a local on-site clinical resource and support home care needs.
• Collaborate with staffing coordinators regarding staffing and scheduling within scope of practice.
• Adhere to nursing delegation guidelines and agency policies.
• Participate in performance improvement initiatives and review clinical documentation.
