Job summary

UP Health System - Bell seeks a Registered Nurse Case Manager (RN) to join our team. This role supports discharge planning, care coordination, and patient advocacy across the multidisciplinary team.

This full-time exempt position offers 10 x 8-hour day shifts, BLS certification required, Michigan RN license, and a commitment to high-quality patient care.

Qualifications

• Current Michigan RN license and associate degree in nursing; BSN preferred.
• AHA Basic Life Support (BLS) certification.
• Two years of recent acute care clinical experience; Utilization Review/Quality Improvement and/or Case Management experience preferred.

Responsibilities

• Consults with nursing staff and multidisciplinary team regularly to evaluate patient's status and appropriateness of medical care, including admission, length of stay, transfer and discharge.
• Monitors patient and family satisfaction; responds to questions and complaints from patients, family members, and payors regarding care.
• Participates in discharge planning including coordinating patient transfers to other facilities and coordinating community resources; provides discharge education and resource referrals to patients.
• Performs chart review to identify actual or potential issues with service delivery, patient outcomes and satisfaction, compliance, cost, and reimbursement.
• Performs other related duties as assigned or requested.

What we're looking for

Applicants should have a current Michigan RN license and possess an associate’s degree from an accredited nursing school (bachelor’s preferred).

Additional requirements include:

• Certifications: AHA Basic Life Support (BLS)

• Experience: Two years of recent related acute care clinical experience, Utilization Review, Quality Improvement and/or Case Management (preferred)
