You will develop and oversee each patient’s Plan of Care, coordinate with families and physicians, and ensure timely documentation while promoting independence and satisfaction.

Qualifications

• Minimum 2 years of nursing experience in acute or post-acute care.
• Active State RN licensure required.
• Proof of CPR certification and valid driver's license.

Responsibilities

• Manage patient care from admission through discharge under the Plan of Care.
• Coordinate with patients, families, physicians, and care team members.
• Submit accurate, timely clinical documentation and progress notes.
• Participate in case conferences to review services, documentation, and goals.
• Participate in on-call rotation as assigned.

Job Summary

We are seeking compassionate Registered Nurses (RNs) to join our team as Case Managers, working one-on-one with patients in their home environment. As a Case Manager, you will establish and oversee all aspects of patient care, creating a Plan of Care and following physician orders to achieve outcomes of increased patient independence and satisfaction. Responsibilities

• Assume responsibility for assigned patient's care from admission through discharge, effectively executing patient's Plan of Care
• Collaborate with patients, family/caregiver(s), ordering physicians, other care team members, and community resources
• Submit accurate, complete, timely, meaningful, and required documentation, including progress and clinical notes
• Participate in case conferences with other clinicians, reviewing services, clinical documentation, and discussing pertinent patient care issues and goals
• Participate in on-call rotation as reasonably assigned

Qualifications

• Minimum of 2 years of nursing experience, with a preference for 2+ years in an acute or post-acute care setting
• Active State Registered Nurse licensure required
• Proof of current CPR certification and valid Driver's license
