Job summary

Saint Francis Hospital is seeking a Registered Nurse Case Manager to coordinate patient care, assess needs, and develop care plans in a 12 Hour Day Shift. The role requires an active Connecticut RN license and corresponds to a full-time schedule at Saint Francis Hospital, a Trinity Health Of New England facility.

Strong collaboration with care teams and patient advocacy are essential. Preferred candidates hold a Bachelor's in Nursing with at least five years in acute care or similar settings,

Qualifications

• Five years of diversified, progressive experience in acute care and/or other settings within the continuum of Care including an understanding of nationally recognized medical utilisation criteria, HCFA/OBRA and state of Connecticut regulatory compliance issues, and JCAHO accreditation accountabilities.
• BLS certification required

Responsibilities

• Coordinate patient care, assess needs, and develop care plans.
• Collaborate with healthcare providers, monitor patient progress, and ensure a smooth transition between care stages.
• Advocate for the patients' well-being during all care transitions.

Education

Graduation from an accredited nursing school

Position Purpose

At Saint Francis Hospital our RN Case Manager are responsible for coordinating patient care, assessing needs, and developing care plans. They collaborate with healthcare providers, monitor patient progress, and ensure a smooth transition between different stages of care well advocating for the patients' well-being.

What you will do:

• As a Registered Nurse RN Case Manager Department, you will serve as a driver of a collaborative, process which coordinates, monitors, and plans health care services throughout the patients’ illness.

Minimum Qualifications:

Required:

• Graduation from an accredited nursing school

• Valid RN license in Connecticut

Preferred:

• Bachelor's degree in Nursing
