Job summary

Texas Health Denton is seeking a Registered Nurse Care Transitions Manager (PRN) to oversee timely patient transitions to appropriate care levels. The role requires discharge planning experience and an active RN license.

Onsite at Denton location with varied hours, including weekends and holidays as needed. Responsibilities include evaluating transitions, reviewing readmission risk scores, and arranging post-discharge follow-up with PCPs or specialists.

Qualifications

  • Bachelor's Degree in Nursing required.
  • 3 years of Staff Nurse experience in an acute care hospital.
  • 1 year of discharge planning and/or care management preferred.
  • Active RN license required at hire.

Responsibilities

  • Ensure patients transition to appropriate levels of care in a timely, effective manner.
  • Complete Transition Evaluations and collect SDOH data within 48 hours of identification.
  • Identify PCPs for patients without a PCP and schedule follow-up care prior to discharge.

Work location: Texas Health Denton 3000 N I-35 Denton, TX 76201 (Onsite)

Work hours: PRN (As Needed), schedule can vary depending on the unit; Monday-Friday, 8:30am-5:00pm or 10:00am-10:30pm and Saturday & Sunday, 8:30am-5:00pm. This position requires 2 weekend days/month minimum and 1 critical holiday staffing.

Here’s What You Need
  • Bachelor's Degree Nursing Individuals hired as CTRN prior to May 11, 2017, will be grandfathered to the CTRN position with an RN, at the entity they were employed at on May 11, 2017. Required
  • 3 years of Staff Nurse at an acute care hospital is required.
  • 1 year of discharge planning and/or care management is highly preferred.
  • Must hold an active RN license - Registered Nurse Upon Hire is required.
  • CPR - Cardiopulmonary Resuscitation is required upon hire.
  • ACM - Accredited Case Manager is preferred upon hire.
  • CCM - Certified Case Manager is preferred upon hire.
  • Other ANCC is preferred.
What You Will Do

Responsible for ensuring patients are transitioned to appropriate levels of care in a timely and effective manner. In addition to the required qualifications, a successful RN will:

  • Completes Transition Evaluations and collects Social Determinants of Health (SDOH) data on patients within 48 hours of identification and begins discharge planning. Assesses and interviews patient and caregivers as part of this evaluation and as needed.
  • Reviews the Risk of Unplanned Readmission (RUR) scores daily for all assigned patients.
  • Assists in the identification of a primary care physician (PCP) for patients without a PCP and attempts to schedule follow up appointments with either a PCP, specialist, clinic, physician or other transitional care visit prior to discharge.
