The role involves using the nursing process with at-risk patients to assess, plan, implement, and evaluate clinical strategies that lead to quality outcomes, cost efficiency, and enhanced patient experience. The nurse works across the care continuum to support patients in clinical management, education, psychosocial needs, self-management, advocacy, and care continuity. Establishing therapeutic relationships with patients and families is essential to improve access, reduce disease burden, manage care transitions, and facilitate appropriate care access points.
The nurse facilitates communication among patients, families, multidisciplinary teams, and community resources to identify and resolve barriers to care. They interpret contracts and obligations to optimize clinical and financial results. Collaboration with patients, families, and inter-professional teams ensures delivery of high-quality, efficient, and cost-effective healthcare, either embedded within provider offices or telephonically.
Evidence-based approaches are used to promote patient and family activation and engagement. The nurse partners with patients to develop SMART goals and assists in creating and adopting educational resources. They identify barriers to learning or care delivery, report abnormal findings, and collaborate on plans.
Independently managing caseloads according to department standards, the nurse ensures timely documentation and task completion, and partners with at-risk patients throughout their care journey. They anticipate patient needs, recognize status changes, and prioritize care accordingly. The nurse coordinates information and communication among all care parties to ensure smooth transitions and manages referrals as needed.

Licenses & Certifications:
Registered Nurse license issued by the state of practice. WI and IL RN licensure preferred, with willingness to obtain both.
Degrees:
Bachelor's Degree in Nursing or related field.
Required Functional Experience:
At least 5 years of clinical nursing experience or 1-2 years in care management.
Knowledge, Skills & Abilities:
Care Management/Transition Certification within 2 years of employment unless holding an advanced degree or enrolled in a master's program. Must be self-directed, capable of working independently and in teams, with excellent communication, critical thinking, problem-solving, and organizational skills. Proficiency in Microsoft Office and Epic EMR is required. Ability to handle multiple responsibilities effectively.
Pay Range:
