On our PM shift, you will:

• Maintain and actively promote effective communication within the care team, ensuring a smooth flow of information during shift changes.
• Assure quality of care adheres to best practices and facility standards, contributing to our commitment to excellence.
• Conduct routine checks of residents, monitoring their well-being and addressing any afternoon or evening needs.
• Administer and accurately record all medications as ordered by a physician, with a focus on the PM medication schedule.
• Take and record vital signs with precision, contributing to accurate resident assessments.
• Follow all up-to-date standards of hygiene, safety, and infection control, ensuring a safe environment for residents and staff during the PM hours.
• Monitor residents who self-administer medication and report any deviations from their care plan.
• Remove discontinued, outdated, or deteriorated medications from inventory according to policy.
• Recognize and promptly report any unsafe conditions related to resident care or medication provision.
• Follow all department safety rules to ensure a safe working environment during your shift.
  
  
  

What You'll Need to Succeed on our PM Team:

• Graduate of an accredited School of Nursing.
• Must possess a current and valid LPN License in the state of Wisconsin.
• Demonstrated ability to work cooperatively and effectively within a team, especially during shift transitions.
