We have an exciting opportunity for an experienced Registered Nurse (RN) Care Manager to join our growing Population Health team! The RN Care Manager will work with patients with poorly managed chronic conditions affecting their quality of life, symptom management, and service utilization. The RN Care Manager connects with patients via telehealth, phone, home, or in-practice visits, supporting them through managing complex medical needs such as COPD, HF, CKD, and HTN. As a trusted team member, the RN Care Manager collaborates with patients to set goals that improve quality of life, patient experience, clinical outcomes, and reduce avoidable ED/hospital visits.
The RN Care Manager reviews patient status, provides ongoing education on self-management, identifies Social Drivers of Health (SDOH) barriers, refers to community resources, and coordinates services. This is a full-time, remote position with occasional on-site visits to practices and patient homes.
Summary:
The Registered Nurse - Care Manager manages high-risk patients to promote education, self-management, and timely healthcare delivery, aiming for optimal outcomes and safe patient transfers across the healthcare continuum. The role acts as a bridge between professional staff in care settings and patients/families.
