Job summary

UP Health System - Portage is seeking a Clinic RN to join our team. This full-time position offers 1.0 FTE or 80 hours per pay period, with Monday-Friday day shifts. The role emphasizes delivering high-quality patient care in a supportive nursing team within Lifepoint Health.

Applicants should have an RN license in Michigan (or eligible within 90 days) and an associate degree in nursing; a bachelor’s degree is preferred. BLS is required; vaccination compliance applies.

Qualifications

• Current Michigan RN license or eligibility within 90 days of hire.
• Associate degree from an accredited nursing school; bachelor's degree preferred.
• Basic Life Support certification is required.

Responsibilities

• Performs plan of care interventions including medication administration and clinical treatments.
• Documents patient care and monitors patient condition.
• Coordinates with care team for transfers, discharges, referrals and psychosocial needs.
• Provides patient and family education tailored to age and culture.
• Delegates tasks to qualified staff and handles prior authorizations for tests and meds.

How You'll Contribute

A Clinic RN who excels in this role:

• Performs plan of care intervention, including medication administration, specimen collection, clinical treatments, as well as other medical care treatment.
• Documents patient care given.
• Monitors, records, and communicates patient condition as appropriate to care team, physician, nursing home, patient, and family.
• Collaborates as needed across disciplines to coordinate patient care, including patient transfer, discharge, referral and spiritual/psychosocial support needs.
• Evaluates learning needs of patient and/or family and provides patient/family education appropriate to age, culture, condition, and circumstances.
• Works as an advocate for the physical and emotional well-being of the patient.
• Delegates patient care activities to appropriately qualified staff.
• Performs prior authorizations on tests, procedures, medications, etc. that are required by the patient's insurance.
• Performs other job duties as assigned.

What We're Looking For

Applicants should have a current Michigan RN license (or attain within 90 days of hire) and possess an associate degree from an accredited nursing school (bachelor's degree preferred). Additional requirements include:

• Basic Life Support certification is required (available upon hire)
• Annual Influenza Vaccination or Declination Required
