Job summary

UP Health System - Portage Pointe seeks an RN Admissions Coordinator to support admission operations for the Portage Pointe long-term care facility. The role involves interviewing Elders and families, coordinating internal and external services, and ensuring compliance with treatment plans.

The position requires Michigan RN licensure, an associate degree in nursing, and BLS certification. It offers 8-hour day shifts, a 0.6 FTE schedule, and a comprehensive benefits package to qualified

Qualifications

• Michigan RN license required
• Associate's degree from accredited nursing school (bachelor's preferred)
• Basic Life Support (BLS-HCP) certification
• Influenza vaccination (or declination) annually

Responsibilities

• Interview and assess Elders and family/caregivers/representatives
• Determine and arrange needed internal/external services/interventions
• Participate in case reviews and collaborate with healthcare team
• Perform Medicaid screenings as indicated for NHP
• Maintain Eden Philosophy in daily actions
• Encourage Elders to reach maximum functional ability
• Contribute to staff, Elder, and family development; emphasize teamwork
• Assist in admission process and evaluate nursing needs for placement
• Review applications, manage waitlist, screen referrals
• Complete and ensure accuracy of MDS for all residents
• Stay updated on Medicare criteria and MDS changes
• Attend IDT and QAPI meetings; participate in Family Care Conferences
• Collaborate with MDS Coordinator and Social Worker
• Perform other duties as assigned

Portage Pointe is a long-term care facility with 60 beds to offer to the senior community and is known as the premier nursing home serving the entire Upper Peninsula. We are a certified Eden Alternative facility and take great pride in providing a home‑like environment for residents. Portage Pointe is regulated by the Centers for Medicare and Medicaid (CMS), a member of the American Health Care Association (AHCA), and a member of the Healthcare Association of Michigan (HCAM).

How You'll Contribute

An RN Admissions Coordinator who excels in this role:

• Interviews and assesses Elders and/or Elder's family, caregivers, and/or legal representatives.
• Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.
• Participate in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.
• Performs Medicaid screenings as indicated for NHP.
• Incorporates the Eden Philosophy of living into every action undertaken throughout one's employment.
• Ensures each Elder is encouraged to reach his/her maximum functional ability and helps maintain these abilities.
• Committed to the development of the staff, Elders, and families as collaborative team members. Teamwork is a prime responsibility for everyone.
• Participate in the admission process of prospective residents in terms of their nursing needs and appropriate placement.
• Determine potential expense associated with a potential admission.
• Review all applications for admission, maintain waitlist, screen referrals, act as a resource to the community.
• Complete and assure the accuracy of the MDS for all residents.
• Maintain current working knowledge of Medicare criteria, serving as a resource for nursing staff and communicate changes in regulations.
• Maintain current working knowledge of MDS changes and most updated RAI manual.
• Attends and participates in weekly IDT and QAPI meetings.
• Actively involved in Family Care Conferences.
• Works closely with Facility MDS Coordinator and Social Worker.
• Performs other job duties as assigned.

What We're Looking For

Applicants should have a current Michigan RN license and possess an associate's degree from an accredited nursing school (bachelor's preferred).
