Job summary

UP Health System - Portage, part of Lifepoint Health, seeks an Elder Associate (CNA) to join our care team at Delaware House in Michigan. Engage in compassionate patient care under RN supervision with a focus on safety and accuracy in vital signs, documentation, and daily activities.

Qualifications

• Michigan Nurse Aide certification (CNA) required (must be obtained within 4 months of hire).
• Basic Life Support (BLS) required (available upon hire).
• ServSafe required within 6 months of hire.
• Graduate of a Program in Discipline required.
• High School Diploma or equivalent required.
• Minimum 1–2 years of CNA experience preferred.
• Requires critical thinking, decisive judgment and ability to work with minimal supervision.
• Demonstrates proficiency in verbal and written communication; interacts professionally with all levels.

Responsibilities

• Perform basic patient care under the direction of a Registered Nurse.
• Demonstrate safe care practices for patients.
• Provide general nursing care: positioning, turning, equipment use, bed pan, urinal/commode, ambulation, bathing, hygiene, linens, room maintenance.
• Take and record vitals and intake/output; document timely.

How You’ll Contribute

An Elder Associate who excels at this role:

• Perform basic patient care skills under the direction of a Registered Nurse.
• Demonstrate safe practices with caring for patients.
• Provides general nursing care such as position the patient, lifting, turning, applying/utilizing special equipment, assisting in the use of bed pan, urinal or commode, ambulating the patient, bed bath, oral and hair care, changing bed linens, cleaning and straightening patient room and other general care necessary.
• Takes and records vitals and intake and output measurements accurately. Documents in a timely manner.
• Answers Elder call lights, telephone, and paging system and respond as appropriate.

What We’re Looking For

Portage Pointe is a long‑term care facility with 60 beds to offer to the senior community and is known as the premier nursing home serving the entire Upper Peninsula. We are a certified Eden Alternative facility and take great pride in providing a home‑like environment for residents. Portage Pointe is regulated by the Centers for Medicare and Medicaid (CMS), a member of the American Health Care Association (AHCA), and a member of the Healthcare Association of Michigan (HCAM). UP Health System - Portage has been serving Michigan's Western Upper Peninsula since 1986.
