Job summary

UP Health System - Portage is seeking an Elder Associate (CNA) to provide essential care under RN supervision. The role emphasizes safe patient handling, vital signs, documentation, and responsive communication within a supportive team.

Candidates should have a Michigan CNA, BLS, and ServSafe readiness; a high school diploma or equivalent is required, with 1–2 years CNA experience preferred. This PRN position involves weekend shifts and a minimum four shifts per schedule.

Qualifications

• Michigan Nurse Aide certification (CNA) required (must be obtained within 4 months of hire).
• Basic Life Support (BLS) required (available upon hire).
• ServSafe required within 6 months of hire.
• Graduate of a program in discipline required.
• High School Diploma or equivalent required.
• Minimum 1-2 years CNA experience preferred.
• Ability to work under pressure with minimal supervision.
• Strong verbal and written communication and interpersonal skills.

Responsibilities

• Perform basic patient care skills under the direction of a Registered Nurse.
• Demonstrate safe practices with caring for patients.
• Provide general nursing care such as positioning, lifting, turning, and use of equipment.
• Take and record vitals and intake/output; document promptly.

How You'll Contribute

• Perform basic patient care skills under the direction of a Registered Nurse.
• Demonstrate safe practices with caring for patients.
• Provides general nursing care such as position the patient, lifting, turning, applying/utilizing special equipment, assisting in the use of bed pan, urinal or commode, ambulating the patient, bed bath, oral and hair care, changing bed linens, cleaning and straightening patient room and other general care necessary.
• Takes and records vitals and intake and output measurements accurately. Documents in a timely manner.
• Answers Elder call lights, telephone, and paging system and respond as appropriate.

What We're Looking For

• Applicants must have a Michigan Nurse Aide certification (CNA) required (must be obtained within 4 months of hire) Additional Requirements include:
• Basic Life Support (BLS) required (available upon hire)

Portage Pointe is a long-term care facility with 60 beds to offer to the senior community and is known as the premier nursing home serving the entire Upper Peninsula. We are a certified Eden Alternative facility and take great pride in providing a home-like environment for residents. Portage Pointe is regulated by the Centers for Medicare and Medicaid (CMS), a member of the American Health Care Association (AHCA), and a member of the Healthcare Association of Michigan (HCAM). UP Health System - Portage has been serving Michigan's Western Upper Peninsula since 1896.
