Job summary

UP Health System - Portage is seeking a part-time RN Admissions Coordinator to join Portage Pointe, a 60-bed skilled nursing unit. The role supports admissions, Medicaid screenings, MDS processes, and coordination with the care team on a MWF 8a–4:30p schedule.

The ideal candidate will hold a Michigan RN license with an associate's degree in nursing and embrace Eden philosophy, teamwork, and patient-centered care. Competitive benefits and growth opportunities are offered.

Qualifications

  • Applicants should have a current Michigan RN license and possess an associate's degree from an accredited nursing school (bachelor's preferred).
  • Basic Life Support (BLS) certification and annual influenza vaccination or declination are required.

Responsibilities

  • Interviews and assesses Elders and/or Elder's family, caregivers, and/or legal representatives.
  • Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.
  • Participates in case reviews to evaluate case management and progress; consults with healthcare team to promote compliance with treatment plans.
  • Performs Medicaid screenings as indicated for NHP.
  • Incorporates the Eden Philosophy of living into every action.
  • Ensures each Elder is encouraged to reach maximum functional ability and helps maintain these abilities.
  • Participates in admission process and determines potential expenses.
  • Reviews all applications for admission, maintains waitlist, screens referrals, and acts as a resource to the community.
  • Completes and assures accuracy of the MDS for all residents; maintains knowledge of Medicare criteria and MDS changes.
  • Attends weekly IDT and QAPI meetings and participates in Family Care Conferences.
  • Works closely with Facility MDS Coordinator and Social Worker.

Portage Pointe is a long-term care facility with 60 beds to offer to the senior community and is known as the premier nursing home serving the entire Upper Peninsula. We are a certified Eden Alternative facility and take great pride in providing a home-like environment for residents. Portage Pointe is regulated by the Centers for Medicare and Medicaid (CMS), a member of the American Health Care Association (AHCA), and a member of the Healthcare Association of Michigan (HCAM).

How you'll contribute

An RN Admissions Coordinator who excels in this role:

  • Interviews and assesses Elders and/or Elder's family, caregivers, and/or legal representatives.
  • Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.
  • Participate in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.
  • Performs Medicaid screenings as indicated for NHP.
  • Incorporates the Eden Philosophy of living into every action undertaken throughout one's employment.
  • Ensures each Elder is encouraged to reach his/her maximum functional ability and helps maintain these abilities.
  • Committed to the development of the staff, Elders, and families as collaborative team members. Teamwork is a prime responsibility for everyone.
  • Participate in the admission process of prospective residents in terms of their nursing needs and appropriate placement.
  • Determine potential expense associated with a potential admission.
  • Review all applications for admission, maintain waitlist, screen referrals, act as a resource to the community.
  • Complete and assure the accuracy of the MDS for all residents.
  • Maintain current working knowledge of Medicare criteria, serving as a resource for nursing staff and communicate changes in regulations.
  • Maintain current working knowledge of MDS changes and most updated RAI manual.
  • Attends and participates in weekly IDT and QAPI meetings.
  • Actively involved in Family Care Conferences.
  • Works closely with Facility MDS Coordinator and Social Worker.
  • Performs other job duties as assigned.

What we're looking for
