Job summary

St. Joseph Hospice is hiring a Full-Time Registered Nurse (RN) in Thibodaux, LA. A $7,500 sign-on bonus is offered. The RN will provide compassionate hospice care, perform assessments, and collaborate with the care team to develop and implement the Plan of Care.

Qualifications include LA RN licensure, CPR, EMR knowledge, and 2+ years in hospice or related settings. Valid driver’s license and evidence of insurance are required.

Qualifications

• Must hold a Louisiana RN license.
• Two years RN experience in hospice, oncology, acute care or home health.
• Current CPR certification required.
• Knowledge of EMR systems required.

Responsibilities

• Perform initial and ongoing patient assessments.
• Develop and implement the Hospice Plan of Care.
• Educate patient and family on medications and care tasks.
• Serve as liaison between physician, patient/family and hospice team.
• Maintain accurate medical records and on-going care documentation.
• Participate in on-call schedule and comply with policies.

Qualifications:

• Registered Nurse (RN) license required in State of Louisiana
• Minimum of two (2) years experience as an RN, either in oncology, an acute care setting with direct patient care, a long-term care facility, hospice or home health care. Two (2) of experience in hospice care as an LPN may be substituted for the required 2 years of experience as an RN
• Current CPR certification required
• Current automobile insurance and valid driver’s license
• Knowledge of EMR is required
• Previous Hospice experience preferred
• Compassionate RN with great time management skills

Job Duties:

• Performs the initial and on-going assessment of the patient
• Assist in developing and implementing the Hospice Plan of Care
• Educate patient and family on Plan of Care, including medication administration, dressing changes, oxygen administration, Hospice philosophy and care of the terminally ill patient
• Act as primary liaison between physician, patient/family and hospice team
• Maintain an accurate and updated medical record
• Makes routine supervisory visits
• Participates in agency on‑call schedule
• Abide by company policies and procedures, compliance guidelines, applicable laws, and regulations promptly reporting any suspected violation of compliance standards
