Job summary

St. Joseph Hospice in Thibodaux, LA is hiring a Full-Time Hospice RN. The role requires an active Louisiana RN license, CPR certification and 2+ years of RN experience in hospice/oncology/acute care. A company ESOP ownership option accompanies a comprehensive benefits package.

The RN will assess patients, implement the Hospice Plan of Care, educate families, liaise with physicians, and maintain records. On-call duties and adherence to policies are essential.

Qualifications

• Registered Nurse (RN) license required in State of Louisiana.
• Minimum of two (2) years experience as an RN, in oncology/acute care/long-term care/hospice or home health care; two years in hospice as an LPN may substitute.
• CPR certification required.
• Current automobile insurance and valid driver’s license.
• Knowledge of EMR is required.
• Previous Hospice experience preferred.

Responsibilities

• Performs the initial and ongoing assessment of the patient.
• Assist in developing and implementing the Hospice Plan of Care.
• Educate patient and family on Plan of Care, including medications, dressing changes, oxygen, hospice philosophy and care of the terminally ill patient.
• Act as primary liaison between physician, patient/family and hospice team.
• Maintain an accurate and updated medical record.
• Makes routine supervisory visits.
• Participates in agency on-call schedule.
• Comply with policies, laws and regulations.

Job Duties:
