Job summary

St. Joseph Hospice in Lafayette, LA is hiring a Full-Time Hospice Registered Nurse (RN) to join our patient-centered care team. You will perform initial and ongoing assessments, help develop the Plan of Care, and educate patients and families on hospice care.

Qualified candidates must hold a Louisiana RN license, CPR certification, and have 2+ years of relevant bedside or hospice experience; EMR knowledge preferred. We offer ESOP, competitive pay, benefits, and opportunities for advancement.

Qualifications

• Louisiana RN license required.
• 2+ years experience as RN in hospice/home care/acute care settings.
• CPR certification required.
• Valid driver’s license and auto insurance.
• Knowledge of EMR systems preferred.

Responsibilities

• Perform initial and ongoing patient assessments.
• Assist in developing and implementing the Hospice Plan of Care.
• Educate patient and family on Plan of Care and medications.
• Serve as liaison between physician, patient/family and hospice team.
• Maintain accurate medical records and supervise visits.
• Participate in on-call schedule and ensure compliance with policies.

Qualifications

• Registered Nurse (RN) license required in State of Louisiana
• Minimum of two (2) years full-time experience as an RN, either in oncology, an acute care setting with direct patient care, a long-term care facility, hospice or home health care. Two (2) of experience in hospice care as an LPN may be substituted for the required 2 years of experience as an RN
• Current CPR certification required
• Current automobile insurance and valid driver’s license
• Knowledge of EMR is required
• Previous Hospice experience preferred
• Compassionate RN with great time management skills

Job Duties

• Performs the initial and on-going assessment of the patient
• Assist in developing and implementing the Hospice Plan of Care
• Educate patient and family on Plan of Care, including medication administration, dressing changes, oxygen administration, Hospice philosophy and care of the terminally ill patient
• Act as primary liaison between physician, patient/family and hospice team
• Maintain an accurate and updated medical record
• Makes routine supervisory visits
• Participates in agency on-call scheduleAbide by company policies and procedures, compliance guidelines, applicable laws, and regulations promptly reporting any suspected violation of compliance standards
