Job summary

Elara Caring is seeking a Registered Nurse Home Health in Westlake, OH to deliver high-quality care in patients’ homes. The RN will assess, plan, and provide nursing care per physician orders, while educating families and coordinating with the care team.

The role offers a $5,000 sign-on bonus and emphasizes privacy, safety, and adherence to HIPAA in a home-based setting. Experience in home health is preferred but not required.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited Nursing program (Certificate, Diploma, Associate or Baccalaureate).
• 1+ year of nursing experience in a clinical setting.
• Home Health experience preferred but not required.
• Ability to lift up to 50 pounds and assist with patient transfers.
• Must have reliable transportation or valid driver’s license to travel between patient homes.

As a Registered Nurse Home Health , you’ll contribute to our success in the following ways:

• Assessing patients to determine their care needs.
• Completing initial and ongoing patient assessments.
• Protecting patient and staff privacy in accordance with HIPAA.
• Providing nursing care according to the plan of care and physician orders.
• Monitoring patient conditions and reporting changes to the care team.
• Educating patients and families on their care and safety.
• Taking appropriate action when patient conditions change.
• Helping ensure quality, safe, and continuous patient care.

What is Required?

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Home Health experience is preferred, but not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
