Job summary

Civic Center Health and Rehabilitation is seeking a skilled Registered Nurse (RN) to provide direct nursing care to residents in a long-term care and rehabilitation setting. The RN will help with rehabilitation plans and collaborate with the clinical team to ensure exceptional care.

Qualifications

Responsibilities

• Deliver patient-centered nursing care to residents in long-term care and rehabilitation.
• Assess, monitor, and document residents' health conditions and progress.
• Administer medications and treatments per facility protocols.
• Collaborate with physicians, therapists, and other professionals on individualized care plans.
• Educate residents and families about care plans and health conditions.
• Maintain accurate documentation in line with state and federal regulations.
• Foster a safe, comfortable, and therapeutic environment for residents.
• Supervise and mentor nursing assistants to ensure quality care.
