Job summary

Civic Center Health and Rehabilitation is seeking a skilled Registered Nurse (RN) to provide direct nursing care in a long-term care and rehabilitation setting. The RN will assist with rehabilitation plans and collaborate with the clinical team to uphold high standards of care.

Responsibilities include medication administration, patient assessments, and coordination with physicians and therapists. Strong geriatric care knowledge and excellent communication are essential.

• Provide education and support to residents and their families regarding care plans and health conditions.
• Maintain accurate and timely documentation in compliance with state and federal regulations.
• Foster a safe, comfortable, and therapeutic environment for residents.
• Supervise and mentor nursing assistants to ensure quality care delivery.
