Job summary

Catholic Health Home Care is hiring registered nurses to provide comprehensive home health care in a team-based setting on Long Island. You will assess, plan, and implement patient-specific care plans, monitor vitals, and perform wound care as part of skilled nursing and rehabilitative services.

You’ll work with physicians, therapists, social workers, and home health aides to support an assigned caseload, while educating families on disease management and preventive health.

Qualifications

• New York State RN license required.
• Minimum of one year of hands-on RN experience.
• BLS certification required.
• Valid NYS driver's license with reliable transportation.

Responsibilities

• Case manage, assess, plan, and implement home health care for patients.
• Measure vital signs, administer IVs, change dressings and wound care.
• Provide skilled nursing care and rehabilitative procedures with diverse patient populations.
• Collaborate with physicians, therapy, social services, and home health aides for caseload.
• Educate patients and families on disease management and health promotion.

• 5x 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends

Job Details

Our Home Care RN’s:

• Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.
• Measure vital signs, Administer IVs, change dressings, clean wounds as needed
• Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.
• Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.
• Provide patient and family education for disease management and health promotion as directed by physician’s plan of treatment.
• Provide care to the most upmost standard

Minimum Qualifications:

• New York State RN license
• Minimum of one year of recent hands on experience as a Registered Nurse.
• Basic Life Support Certification
• Valid New York State Driver’s License and reliable transportation

Salary Range

USD $100,000.00 - USD $130,000.00 /Yr.

This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
